Form Usability - Explain why you are asking
Put in a short sentence to tell your visitors why you need the information
You should consider explaining on your form when:
- the forms ask for unfamiliar data
- people question why they are being asked for specific data
- people may be concerned about the security or privacy of their data
This form get it right. Example, refer to the Age and Postal Address field
